Job Summary

The Physiotherapist will be a member of the Interprofessional Primary Care Team and provides client-centered, evidence-informed physiotherapy services to individuals receiving care through the Primary Care Clinic, our community partners. The Physiotherapist works collaboratively with clients and an interdisciplinary team to assess, treat, educate, and support individuals experiencing mobility, functional, and physical health challenges. The role focuses on improving functional independence, preventing injury (e.g. Falls), managing acute and chronic conditions, and enhancing overall quality of life. Services are delivered through individual and group interventions, health promotion activities, community outreach, and care coordination, with a commitment to health equity and improving access to care for diverse and underserved populations.

 

Job Specific Responsibilities:

  • Using a client centered approach, the Physiotherapist assesses the client mobility challenges, establishes treatment goals with the client, and formulates and implements a plan in collaboration with the interprofessional primary care team.
  • Provides education and outreach to clients including appropriate assessments, screening, referral, education, follow up and other clinical activities consistent with the College of Physiotherapists of Ontario standards of physiotherapy practice.
  • Keeps complete, accurate and timely records of client care.
  • Acts as a health resource and provides appropriate support and consultation to staff other interprofessional primary care team.
  • Participates in the preparation of new materials based on best practices for client education.
  • Participates in regular chart reviews and case conferences.
  • Provides health promotion activities as appropriate to the position.
  • Participates actively in information sharing within the program, team and other partner agencies.
  • Assists in the collection and analysis of relevant data for administration, planning and evaluation.
  • Works with internal and external service providers as well as relevant community groups.
  • Participates in the development of primary care and health education programs based on client and community identified health needs.

  • Maintaining competence, and where applicable, a professional licence to practice.
  • Supporting the Centre’s student and volunteer placement programs.
  • Promoting awareness of and participation in Centre activities.
  • Contributing to the Centre’s work by participating in meetings and committees.
  • Working during both regular and extended hours of operation in locations identified by the Centre.
  • Contributing to the Centre’s practices of hiring, orienting and training of staff.
  • Contributing to the Centre’s efforts to secure and maximize resources for current and new programs, services and activities.
  • Participating in the Centre’s efforts to enhance its capacity through staff development.
  • Working in a manner that supports the Centre’s Occupational Health and Safety policies and procedures.

Qualifications:

  • Master’s Degree in Physiotherapy.
  • Current (and in good standing) Registration with the College of Physiotherapists of Ontario.
  • Minimum of three years of Physiotherapy experience in a community setting.
  • Demonstrated commitment to and knowledge of community-based health care.
  • Self-motivated, organized, and able to work independently while collaborating effectively with a team.
  • Demonstrated ability to work in a multi-disciplinary team environment.
  • Demonstrated experience or strong interest in working with one or more of PQCHC’s priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
  • Technology proficiency is an asset PS Suite EMR, Microsoft Office Suite.

  • Excellent communication and interpersonal skills.
  • Linguistic profile: Bilingual English/French is considered a strong asset  
  • Other languages are an asset.
