Founded by Urologist Dr. Alaina Garbens (MD, PhD, FRCSC), Élevé is a physician-led, evidence-based women’s health and regenerative medicine practice bridging the gap between pelvic care, sexual wellness, hormonal health, and regenerative aesthetics.

Élevé @ Form is a new women’s health division within the Form integrated health campus in Midtown Toronto, alongside FORM Face + Body (aesthetics and cosmetic surgery), and FORM’s men’s health program.

The Opportunity

As our pelvic health physiotherapist, you will be working as part of a defined clinical pathway system, where physiotherapy is integrated into treatment plans that include medical assessment, regenerative procedures, hormonal management, and energy-based therapies. Your patients will be referred directly from Élevé’s NP and Dr. Garbens. Every referral comes with a clinical context, a treatment pathway, and a defined role for physiotherapy within that plan.

You will treat women and men across pelvic floor dysfunction, incontinence, postpartum recovery, sexual health, pelvic pain, and pre/post-procedure rehabilitation. Hands-on education and training regarding our innovative treatment plans, which use regenerative and energy-based therapy in your treatment plans alongside manual therapy, exercise prescription, and patient education.

This role is ideal for a highly trained pelvic health physiotherapist who wants clinical mentorship from a urologist, access to advanced technology, and interest in collaboration and research.

What You Will Do

Clinical Care

  • Deliver pelvic health physiotherapy within Élevé’s structured clinical pathways, your role is defined within each pathway, with clear integration points and treatment goals

  • Perform comprehensive pelvic floor assessments, including internal examination, muscle function testing, and functional evaluation

  • Deliver individualized treatment plans incorporating manual therapy, pelvic floor retraining, exercise prescription, behavioural strategies, and patient education

  • Integrate advanced technologies into treatment where clinically appropriate. Treat across the full scope of pelvic health: pelvic floor dysfunction, stress and urge incontinence, pelvic organ prolapse, postpartum recovery, sexual dysfunction, pelvic pain, dyspareunia, and pre/post-procedure rehabilitation

  • Document all patient encounters thoroughly, tracking outcomes and progression within the EMR

Collaboration & Pathway Integration

  • Receive patient referrals from the NP and Dr. Garbens with clinical context and pathway assignment, you will know why each patient is coming to you.

  • Communicate treatment progress and clinical findings back to the referring clinician at defined pathway checkpoints

  • Participate in team huddles and clinical reviews to coordinate patient care across the division

  • Collaborate with the men’s health program on cross-referral patients, including couples presenting with shared pelvic health concern

  • Contribute clinical input to pathway refinement, your expertise will help shape how physiotherapy integrates into Élevé’s evolving programs

Education & Research

  • Lead and co-facilitate patient education workshops and community events on pelvic health topics
  • Support patient education within treatment sessions, help patients understand their condition, their progress, and their role in recovery
  • Contribute to structured clinical outcome data collection from Day 1, this data will support future research and publications
  • Opportunity to participate in research studies under Dr. Garbens’ direction as the division’s research program develops

What We Are Looking For

  • Registered physiotherapist in good standing with the College of Physiotherapists of Ontario
  • Completed Level 1, Level 2, and Level 3 Pelvic Health Solutions (PHS) courses or equivalent,  you must be fully trained and rostered to perform internal pelvic examinations from Day 1
  • A genuine interest and comfort level in treating both women’s and men’s pelvic health,  this is a core focus at the Midtown location
  • Experience or strong interest in working within a multidisciplinary medical team rather than in solo practice

Qualifications: 

  • Physiotherapy degree (MSc or BSc) or University degree in a related field (e.g. public health, health policy, public or business administration) 
  • Understanding of the roles and inter-relationships of stakeholders in the Ontario health system 
  • Knowledge of policy analysis, development and implementation methodology 
  • Qualitative and quantitative research/evaluation skills

Location

Unit Summary

As one of 4 critical care units at the HGH site, Neuro Trauma ICU, a 14 bed Level 3 ICU provides highly skilled tertiary and quaternary care to a population of over 2.3 million residents within the Central South region of Ontario. It supports the regional trauma, neurosurgical and stroke programs in addition to offering specialized care for clinical programs including neurosciences, surgery and spine. Using a multidisciplinary and collaborative team approach that is dedicated to the principles of patient-and family-centered care, we treat highly complex patients using leading edge treatment modalities. In partnership with Trillium Gift of Life, we are leaders in organ donation for the province. The ICU also supports the Critical Care Response Team for the HGH site, bringing highly skilled, specialized, critical care nurses, as members of a multidisciplinary team, to offer support and consultation outside of the ICU. As a Royal College Accredited training program and through our affiliation with McMaster University, we offer opportunities for learning and conduct world renowned research.

Position Summary
The Physiotherapist provides assessments and interventions for a variety of patients in the Critical Care Program. This position is specific to the Neuro Trauma ICU, providing additional coverage to the Cardiovascular ICU and Medical Surgical ICU on weekends. The Physiotherapist participates as a member of an interdisciplinary team and participates in program development, evaluation and research.
