Position Overview:

The physiotherapist (PT) plays an integral role in delivering primary care services within a multi-disciplinary team. Working in collaboration with a team of physicians, nurse practitioners and other allied health professionals, the physiotherapist provides comprehensive assessments, evidence-based treatments and patient education to improve function and quality of life for patients across the lifespan with a focus on individuals that do not have access to physiotherapy supports in the community.

The PT will diagnose within their scope of practice and establish and implement evidence-informed treatment plans that include integration with existing community services. A strong knowledge of community-based programs is required to facilitate clients’ access to external services to meet individual needs and manage caseload demands.

Roles and Responsibilities:

Primary Care Integration

Attachment Efforts:

Support patients who do not have primary care to be referred to the attachment team.

Access Efforts:

Be an ambassador to the groups and workshops offered by HFHT by being aware of current offerings, handing out promotional materials and promoting the groups and workshops to existing, new and future patients.
Contribute to improved access and continuity of care across local healthcare services.

Quality & Performance Improvement

Ensure continuous quality improvements and adherence to best practices or leading practices related to physiotherapy and primary healthcare services, particularly in relationship to priority populations.
Ensure services and programs follow ministry guidelines, HFHT policies, legislation and professional standards
In collaboration with the team, participate in program planning, development and evaluation of physiotherapy services for individuals and within groups and workshops, as appropriate.
Monitor data, patient outcomes and satisfaction to inform service provision improvements.

Patient Experience

Foster a welcoming, respectful, and inclusive care environment.
Address patient concerns with patient-centeredness and professionalism.
Foster a culture of patient engagement and shared decision making

Resource Optimization

Support the effective use of resources to meet service demands

Physiotherapy related Duties and Responsibilities:

Conduct musculoskeletal, neurological, and functional assessments.
Develop and implement individualized treatment plans.
Provide hands-on therapy, exercise prescription, education, and self-management strategies including encouragement to attend HFHT groups such as GLAD, Just Walk or other relevant programs.
Collaborate with other team members to support chronic disease management, falls prevention and other preventative strategies.
Maintain accurate, timely documentation in the electronic medical record (EMR) and communicate consult notes to the primary care team as needed.
Assist patients with navigation of healthcare and community support systems.
Collaborate with interprofessional team members to support coordinated, patient-centered care.

Qualifications and Skills

Required:

Masters of Science in Physiotherapy MSc (PT) or equivalent
Registered and in good standing with the College of Physiotherapists of Ontario
Up to date professional liability insurance, membership of Canadian Physiotherapy Association / Ontario Physiotherapy Association.
Proficiency completing clinical assessments and evidence-based care plans for patients with complex conditions while working with them to develop self-management skills.
Experience and competency supporting individuals with chronic pain, musculoskeletal injuries, and vestibular conditions
Excels at building effective, professional relationships with patients, provide a supportive approach to care and guide patients to work towards independence.
Demonstrated clinical reasoning and therapeutic skills
Strong communication and documentation skills
Proficiency with EMR systems (Telus PS) and virtual care platforms (i.e. EMRs, Microsoft Teams)
Ability to work independently in a collaborative, interdisciplinary setting
Knowledge of relevant clinical practices, legislation and guidelines.

Preferred:

Primary care, community, and team-based care experience is preferred along with a supportive approach to patients
Understanding of quality improvement processes as they relate to patient care and program evaluation.
Strong knowledge of best practices and benchmarks for advancing integrated care.
Experience or interest in supervising student learners from physiotherapy and other health care professions
GLAD certified preferred

Our Team:

The Arthritis Rehabilitation and Education Program (AREP) provides a range of treatment and education services for adults and children with arthritis. Services are free, available in most regions in Ontario, and are funded by the Ontario Government for people with an Ontario Health Card and a confirmed diagnosis of arthritis. Our team of specially trained physiotherapists, occupational therapists and social work professionals will provide people with arthritis with the care, strategies, guidance and education they need to better manage their arthritis and live well using a chronic disease self -management model.

Key Accountabilities:

Utilize advanced rheumatology skills and chronic disease management principles to provide rehabilitation to individuals with arthritis.
Collaboratively establish customized Arthritis Service Plans that address both immediate and long-term needs and goals of patients and their families.
Conduct group education sessions for patients and families, focusing on arthritis self-management and related topics, both in-person and virtually.
May periodically facilitate and support education of other healthcare providers.
Assess the clinical status of the patient within a defined scope of practice.
Facilitate continuity of care among external health professionals and service providers.
Advocate for patient access to arthritis care and resources.
Create organized and accurate patient files with thorough documentation of events, decisions, interventions, decisions, and outcomes in accordance with AREP guidelines and regulatory standards.
Establish, grow and monitor strategic partnerships across external healthcare settings to improve the accessibility, delivery and presence of AREP services.
Provide internal and external education on arthritis, engage in approved clinical program development and evaluation, and participate in quality improvement and research activities.

Qualifications, Experience & Skills:

Master’s degree or equivalent in Occupational Therapy or Physiotherapy.
Member in good standing of the relevant regulatory College of Ontario.
Preferred qualifications include a minimum of 3 years of clinical experience in a busy and progressive ambulatory health environment, experience working with individuals with musculoskeletal and chronic diseases, familiarity with progressive interprofessional settings, previous experience with virtual service delivery platforms, and proficiency in Microsoft Office with preferred experience in scheduling and electronic medical record software. Demonstrated ability to use approved Artificial Intelligence (AI) tools responsibly to enhance productivity, streamline workflows, and improve work output while maintaining accuracy, confidentiality, and professional judgment.

Working Conditions:

Physiotherapist

KEY FUNCTION:

Provide safe, effective, and efficient physiotherapy services to the in-patient environments including Medical/Surgical, ICU, ER, CCU units, and St. Joseph’s Manor.

Physiotherapists will be dedicated to improving SJGHEL’s inpatient mobility:

Promoting optimal mobility, physical activity and overall health and wellness.
Preventing injury and disability.
Assessing the impact of injury, disease or disorders of movement and function.
Managing acute and chronic conditions, activity limitations, and participation restrictions through education, specialized programming, and a holistic approach to care.
Restoring, improving, maintaining, and prolonging optimal functional independence and physical performance.
Educating and planning maintenance and support programs to prevent re-occurrence, re-injury or functional decline.
Completing and submitting applications for patients to access appropriate rehabilitation programs and community resources, ensuring accuracy and timely processing.
Collaborating with the interdisciplinary team to develop individualized care plans and actively participating in discharge planning to ensure safe, effective transitions and continuity of care.

DUTIES:

Perform detailed, comprehensive medical history and physical assessment of patients with actual or potential impairments, pain, functional limitations, disabilities, or other health related conditions to establish a diagnosis and clinical impression resulting from assessment findings.
Use professional clinical reasoning to develop individualized treatment plans for all patients based on their abilities, functional needs, and potential for functional change.
Communicate the plan of treatment to patients and the interdisciplinary team.
Obtain informed consent from each patient and/or their substitute decision maker for the treatment plan and for the involvement of Rehabilitation Assistants or students.
Provide supervision and work direction to Rehabilitation Assistants and assigned support personnel as required in accordance with the standards defined by the CPO.
Initiate and implement all physiotherapy treatments related to the diagnosis and clinical impression.
Operate a variety of equipment to complete patient therapy and ensure maximum care and safety of patients during treatment.
Recommend and educate compensatory and remedial strategies, activity modification, adaptive equipment, and assistive devices to promote safety and maximize functional independence.
Monitor patient progress, perform on-going re-evaluations and initiate program changes as required.
Collaborate with patients, families, physicians, interprofessional team members and community partners throughout the therapy process regarding patient management.
Educate, motivate, and counsel patients and their families in coping strategies, decision-making and utilization of personal and community resources as needed to ensure successful completion of programs.
Make referrals to regional/provincial specialized rehabilitation programs as required.
Participate in weekly interdisciplinary rounds and ALC avoidance rounds.
Participate in the discharge planning process to ensure safe, effective, and efficient discharges.
Make referrals to local community partners when discharging patients to ensure appropriate follow-up therapy services are in place as needed.
Document assessment findings, clinical impression, treatment plans, and all related physiotherapy intervention and patient outcomes. Maintain an accepted standard of documentation for quality assurance.
Ensure the privacy and confidentiality of personal health information is maintained.
Act as a professional resource for internal health care providers and community partners.
Participate in program planning committee work as required.
Provide theoretical and clinical education/supervision to physiotherapy students and undergraduate students.
Complete mandatory education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines as determined by SJGHEL.
Provide in-services and education to the community, hospital staff members, patients and family members as needed (ie: Back Care Class)
Educate and promote health, safety and wellness in the workplace and adhere to hospital safety and infection control measures.
Maintain relevant statistical information.
Perform other duties as assigned.

QUALIFICATIONS

EDUCATION AND TRAINING:

Master’s Degree in Physiotherapy from an accredited Canadian university (or equivalent internationally).
Registration and licensing in good standing with the College of Physiotherapists of Ontario (CPO).

EXPERIENCE:

Experience in working with geriatric and adult populations is an asset.
