Insight Health Solutions (IHS), fully part of THP, provides third-party health services, including assessment, rehabilitation, and disability management, to insurers, employers, and WSIB. As a leading provider of hospital-based third-party clinical services for over 25 years, we are committed to delivery of high quality care across Ontario to a broad spectrum of customers and clients.

About the Home Care Program: 

IHS has a contract to deliver home care services for Injured Persons throughout the province of Ontario in collaboration with a select group or regional providers by offering home care services.  The WSIB Home Care Services Program (“HCSP”) provides health care services to Injured Persons within their home. The program supports Injured Persons throughout the health-injury/illness continuum including end-of-life care.  In some cases, the need for care will be short term (e.g., while the Injured Person recovers from a surgery), but in other cases, the care will be required on a long-term basis (over one year), or permanently for Injured Persons with severe physical and/or cognitive injuries.   The goal of the Services is to make it possible for Injured Persons who have sustained a workplace injury or illness to:

  1. Transition from hospital to convalesce safely in-home (hospital-to-home), and/or remain safely in their home functioning at their optimal level of independence; and
  2. Promote injury/illness recovery and/or support the Injured Person to achieve the highest quality of life possible.

Position Description: 

Insight Health Solutions is seeking interested Physiotherapists with home care experience to provide home care services in their local communities across Ontario.

Reporting to the Manager, Insight Health Solutions, the Physiotherapists will work collaboratively with an inter-professional team, both internal and external to THP which includes the WSIB, subcontractor agencies, family members and support networks.

The Physiotherapists will identify and facilitate rapid access to care that enables discharge home from the hospital and/or enhanced recovery and return to work in the home.  Compensation will be offered on a fee for-service basis and for travel over 60km at a pre-determined rate.

Responsibilities:

  • Deliver Physiotherapy assessment and treatment in the home for injured persons who cannot access treatment on an out-patient (in-clinic) basis
    • Initial assessment may include a home safety assessment or a physical examination
  • Provide education and self-management techniques and coping strategies to address any outstanding concerns to promote participation in activities of daily living and independence
  • Provide consistent messaging related to psychosocial management to reinforce learnings and facilitate transfer to specific and broader activities of daily living, work and leisure
  • Educate and delegate to other regulated and unregulated health care team members, as appropriate (e.g. tasks such as using transfer equipment to transfer the Injured Person or how to assist the Injured Person with a home exercise protocol)
  • Educate the Injured Person on how to perform a home exercise protocol, as appropriate for injury type to promote recovery, independence, and/or quality of life
  • Adapt a care plan in response to the Injured Person’s changing needs and progress through the course of the treatment block. The treatment plan is to include an active treatment plan, education, exercise, and, where appropriate, progression of care
  • Active and timely communication with IHS during the program when an Injured Person is ready for in-clinic intervention, and/or if they are not progressing
  • Provide a Rehabilitation Initial Intervention Report, or Rehabilitation Progress/Discharge Report following an assessment of the of the Injured Person to the Care Coordinator in the required time frame
  • Provide treatment interventions that may include but are not limited to:
    • maintain or increase mobility, range of motion (ROM) (e.g. contracture prevention), strength, muscle function, balance and/or coordination
    • activities of daily living training, functional transfer training, sleep hygiene, cognitive/behavioral activation, adaptive communication, sensory regulation, falls prevention
    • provide recommendations on required equipment
    • respiratory health
    • neurological rehabilitation
    • gait training
    • chronic and palliative condition management (quality of life); and
    • pain management, including education on proactive self-management strategies.
  • Collaborate with and engage in regular communication with IHS’ Care Coordinator to ensure there is clear and objective information when a recommendation is made to change the Injured Person’s level of care
  • Participate in case conferences with the care team and WSIB if requested
  • Support transition planning with other homecare and community support providers where needed if an Injured Person is referred for services while still in an acute care facility and/or intensive inpatient rehabilitation
  • Communicate assessment and treatment reports with primary care providers to ensure the Injured Person has necessary medical oversight and a complete care plan while supported at home
  • Communicate with the IHS Care Coordinator to advise on any material changes to the Injured Person’s health status, complaints or concerns
  • Advise IHS of necessary medical goods/equipment when indicated
  • Foster relationships with other subcontracted providers in the home to facilitate continuity of patient care
  • Communicate and collaborate with Injured Persons, families, members of the health care team, and community resources to support decision making regarding appropriate services
  • Collect data for reporting, monitoring and evaluation as required related to service delivery
  • Must have your own vehicle
  • Exceptional written and verbal communication skills

Key Qualifications:

  • Physiotherapist Registered for Independent Practice in good standing with the College of Physiotherapists of Ontario
  • Registered Physiotherapist with a minimum of three years of clinical experience delivering home care as a registered Physiotherapist in home community health care setting required
  • Fluency in English (verbal and written) is required
  • Fluency in French (verbal and written) is preferred but not required
  • ADP authorized for equipment provision is an asset
  • Computer literacy and proficiency is required and access to a secure computer for receive referrals and submit reports is required 
  • Comprehensive knowledge of community resources and health care delivery systems in acute and community settings required 
  • Excellent time management, interpersonal, communication, organizational and problem-solving skills required
  • Demonstrated ability to work independently and collaboratively with an interdisciplinary team, family members, support networks, and a variety of formal and informal service providers
  • Demonstrated commitment to patient centered holistic care and demonstrated ability to respond to patients with flexibility and adaptability
  • Positive attendance record is required
  • Evidence of a clear Criminal Record check as defined but the Police Record Check Reform Act, 2015 (the “PRCRA”) or in addition to the screening listed above, an out-of-country police certificate (or delivery of a written declaration) for anyone who has lived outside of Canada for more than six (6) consecutive months in the last five (5) years.

We have  physiotherapists, massage therapists and pilates instructors on staff.  We are looking to add another Pelvic Floor Physiotherapist to our dynamic team.

  2. Regular support in building and maintaining your caseload
  3. $1,000 Education allowance per year
  4. Vacation pay ($)

  7. No admin work. We manage all reception, appointments, invoicing, and billing for you
  8. Clinical Mentorship – for new grads or clinicians looking to upgrade their skills and certifications.  We have 3 FCAMPT therapists, Pelvic Physiotherapist, and Vestibular Physiotherapist on staff that are available for mentorship opportunities!!

Whether you are a season clinician or new grad, we help you plan your certification and (new) programs to sharpen and add new skills to your clinical tool-kit. As clinicians our learning never ends.

As a new grad you can also take advantage of our Direct Mentorship Program. The Direct Mentorship Program is on-going until you feel like a clinician superstar.

How does it work? What does it include?

  • You will meet regularly with a dedicated clinical mentor
  • Practice new and advance techniques to build and sharpen your skills
  • Course and certification planning
  • Get help to build a full caseload and maintain it
  • Learn how to create a treatment plan and keep patients committed to it so they graduate and get their outcomes

Required Competencies:

  • Registered member (in good standing) with the College of Physiotherapists of Ontario
  • Pelvic Floor Training
