Physiotherapist

KEY FUNCTION:

Provide safe, effective, and efficient physiotherapy services to the in-patient environments including Medical/Surgical, ICU, ER, CCU units, and St. Joseph’s Manor.

Physiotherapists will be dedicated to improving SJGHEL’s inpatient mobility:

Promoting optimal mobility, physical activity and overall health and wellness.
Preventing injury and disability.
Assessing the impact of injury, disease or disorders of movement and function.
Managing acute and chronic conditions, activity limitations, and participation restrictions through education, specialized programming, and a holistic approach to care.
Restoring, improving, maintaining, and prolonging optimal functional independence and physical performance.
Educating and planning maintenance and support programs to prevent re-occurrence, re-injury or functional decline.
Completing and submitting applications for patients to access appropriate rehabilitation programs and community resources, ensuring accuracy and timely processing.
Collaborating with the interdisciplinary team to develop individualized care plans and actively participating in discharge planning to ensure safe, effective transitions and continuity of care.

DUTIES:

Perform detailed, comprehensive medical history and physical assessment of patients with actual or potential impairments, pain, functional limitations, disabilities, or other health related conditions to establish a diagnosis and clinical impression resulting from assessment findings.
Use professional clinical reasoning to develop individualized treatment plans for all patients based on their abilities, functional needs, and potential for functional change.
Communicate the plan of treatment to patients and the interdisciplinary team.
Obtain informed consent from each patient and/or their substitute decision maker for the treatment plan and for the involvement of Rehabilitation Assistants or students.
Provide supervision and work direction to Rehabilitation Assistants and assigned support personnel as required in accordance with the standards defined by the CPO.
Initiate and implement all physiotherapy treatments related to the diagnosis and clinical impression.
Operate a variety of equipment to complete patient therapy and ensure maximum care and safety of patients during treatment.
Recommend and educate compensatory and remedial strategies, activity modification, adaptive equipment, and assistive devices to promote safety and maximize functional independence.
Monitor patient progress, perform on-going re-evaluations and initiate program changes as required.
Collaborate with patients, families, physicians, interprofessional team members and community partners throughout the therapy process regarding patient management.
Educate, motivate, and counsel patients and their families in coping strategies, decision-making and utilization of personal and community resources as needed to ensure successful completion of programs.
Make referrals to regional/provincial specialized rehabilitation programs as required.
Participate in weekly interdisciplinary rounds and ALC avoidance rounds.
Participate in the discharge planning process to ensure safe, effective, and efficient discharges.
Make referrals to local community partners when discharging patients to ensure appropriate follow-up therapy services are in place as needed.
Document assessment findings, clinical impression, treatment plans, and all related physiotherapy intervention and patient outcomes. Maintain an accepted standard of documentation for quality assurance.
Ensure the privacy and confidentiality of personal health information is maintained.
Act as a professional resource for internal health care providers and community partners.
Participate in program planning committee work as required.
Provide theoretical and clinical education/supervision to physiotherapy students and undergraduate students.
Complete mandatory education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines as determined by SJGHEL.
Provide in-services and education to the community, hospital staff members, patients and family members as needed (ie: Back Care Class)
Educate and promote health, safety and wellness in the workplace and adhere to hospital safety and infection control measures.
Maintain relevant statistical information.
Perform other duties as assigned.

QUALIFICATIONS

EDUCATION AND TRAINING:

Master’s Degree in Physiotherapy from an accredited Canadian university (or equivalent internationally).
Registration and licensing in good standing with the College of Physiotherapists of Ontario (CPO).

EXPERIENCE:

Experience in working with geriatric and adult populations is an asset.

KNOWLEDGE/SKILLS/ABILITIES:

PHYSICAL DEMAND:

Ability to remain on your feet for extended periods, including standing and walking.
Frequent repetitive movements associated with assisting patients and handling equipment.

Our Team:

The Arthritis Rehabilitation and Education Program (AREP) provides a range of treatment and education services for adults and children with arthritis. Services are free, available in most regions in Ontario, and are funded by the Ontario Government for people with an Ontario Health Card and a confirmed diagnosis of arthritis. Our team of specially trained physiotherapists, occupational therapists and social work professionals will provide people with arthritis with the care, strategies, guidance and education they need to better manage their arthritis and live well using a chronic disease self -management model.

Key Accountabilities:

Utilize advanced rheumatology skills and chronic disease management principles to provide rehabilitation to individuals with arthritis.
Collaboratively establish customized Arthritis Service Plans that address both immediate and long-term needs and goals of patients and their families.
Conduct group education sessions for patients and families, focusing on arthritis self-management and related topics, both in-person and virtually.
May periodically facilitate and support education of other healthcare providers.
Assess the clinical status of the patient within a defined scope of practice.
Facilitate continuity of care among external health professionals and service providers.
Advocate for patient access to arthritis care and resources.
Establish, grow and monitor strategic partnerships across external healthcare settings to improve the accessibility, delivery and presence of AREP services.
Provide internal and external education on arthritis, engage in approved clinical program development and evaluation, and participate in quality improvement and research activities.

Qualifications, Experience & Skills:

Master’s degree or equivalent in Occupational Therapy or Physiotherapy.
Member in good standing of the relevant regulatory College of Ontario.
Preferred qualifications include a minimum of 3 years of clinical experience in a busy and progressive ambulatory health environment, experience working with individuals with musculoskeletal and chronic diseases, familiarity with progressive interprofessional settings, previous experience with virtual service delivery platforms, and proficiency in Microsoft Office with preferred experience in scheduling and electronic medical record software.

Working Conditions:

This is a full-time role (35 hours per week) between Monday and Friday inclusive.
This position is designated to our GTA (including Etobicoke & Downtown Toronto) clinic sites in a hybrid model, with occasional visits to client homes. The exact days you are required to work from these locations will be determined by your supervisor.
As this role will involve travel, regular access to a motor vehicle, a valid driver’s license, and the ability to travel is essential.

Physical Therapy Program Tenure/track position

The School of Rehabilitation Therapy in Queen’s Health Sciences at Queen’s University invites applications for a teaching-focused tenure-track faculty position with teaching responsibilities primarily in the Physical Therapy Program. The position will be at the rank of Assistant Professor. The role will be 70% teaching, with the remaining 30% being allocated to research and service. The preferred start date is July 1, 2027.

Qualifications

Candidates must be registered (or eligible for registration) as a Physical Therapist in Ontario. Candidates must have a PhD or equivalent degree (e.g., EdD, DSc) completed by the start date of the appointment. A DPT degree will be considered provided the applicant has evidence of research training and experience to support educational scholarship. The main criterion for selection is academic excellence in teaching and learning.

Teaching activities will constitute 70% of the role. This position is designed for a person seeking a teaching career in a graduate level entry-to-practice physical therapy program. The role also includes the possibility of teaching and supervising graduate students in other programs in the School of Rehabilitation Therapy.

The ideal candidate has teaching experience and a passion for teaching and learning as evidenced by educational innovation and leadership, curriculum design and delivery, and potential for education research. In particular, the successful candidate must provide evidence of strong potential for outstanding teaching contributions in a graduate level entry-to-practice physical therapy curriculum. The successful candidate will also have experience promoting equity, diversity, inclusion, Indigenization, and/or accessibility through their teaching and learning practices.

Candidates will be passionate about joining a Physical Therapy program that uses a team-based teaching approach within an innovative, competency-based Physical Therapy education program, which was launched in September 2023.

As a competency-based education program, our curriculum is organized around physiotherapy competency domains from the Competency Profile for Physiotherapists in Canada (NPAG, 2017). We are seeking candidates who can provide teaching expertise and leadership in core courses within any of the following competency domains:

Professionalism, Collaboration, Leadership, and Management (e.g. courses: PT811, PT812, PT813)
Scholarship (e.g. courses: PT831, PT832, PT833, PT898)
Physiotherapy expertise (e.g. courses: PT871, PT872, PT873, PT831, PT832, PT833)

While we welcome candidates with expertise and/or experience teaching in any of these areas, preference will be given to candidates with expertise and/or experience in teaching in the professionalism, collaboration, leadership, and management competency domains. Teaching experience on the assessment and management of (1) complex health conditions, (2) neurological conditions and/or (3) pediatric conditions is an asset. Demonstrated engagement with the Physical Therapy Profession (e.g. through involvement in the Canadian Physiotherapy Association) and experience working within Canadian health systems are also assets.

We welcome candidates with research and scholarship expertise in any relevant area; however, preference will be given to candidates whose scholarship focuses on the art and science of teaching and learning. This scholar will join a community of other faculty members conducting education research within the School of Rehabilitation, Queen’s Health Sciences, and the new Queen’s Institute for Educational Scholarship and Research.

The diverse backgrounds of applicants and the type of scholarship appropriate to their research areas, as well as the diverse range of scholarly methodologies and ways of measuring impact will be taken into account when assessing the quality of the scholarly or creative work. Candidates will be evaluated using the principles of the Declaration on Research Assessment (DORA) and with attention to the diverse ways excellence can be demonstrated. The successful candidate will also be expected to make contributions through service to the department, the Faculty, the University, and/or the broader community. The annual salary for this appointment is expected to be between $145,000 and $160,000. Actual Salary will be commensurate with qualifications and experience

In East York we share the building with Resilience Fitness, the top Hyrox gym in the GTA, and there’s a Pilates reformer on site. It’s busy, it’s hands-on, and the culture rewards collaboration and knowledge sharing. We do weekly rounds too and new grads can expect a guided mentorship phase.

Responsibilities:

Provide community-based physiotherapy services within full scope of practices outlined by the College of Physiotherapists of Ontario, including but not limited to assessment, diagnosis and education intervention
Diagnose and assist diverse clients with acute and chronic health conditions and rehabilitation needs, from the lens of chronic disease prevention and management
Provide physiotherapy services independently and within a team, including rehabilitation assistants
Ensuring practice is consistent with professional standards and the ethical framework as outlined by the College of Physiotherapists of Ontario and organizational policy, practice and values
Demonstrating commitment to professional life long learning by participating in continuing education activities to meet the needs of the patient population served and the standards of the profession

Requirements:

Master of Science in Physiotherapy or equivalent
Registration and licensed to practice (in good standing) with the College of Physiotherapy of Ontario
3 years related clinical experience in the community setting
Valid First Aid & CPR certification
Demonstrated capability to plan and recommend continued self-care programs
Strong interpersonal, organizational and problem-solving skills
Shown capacity to make independent judgment and work without supervision
Proficiency in English; a second language reflecting local community, such as Cantonese, is an asset

Physiotherapist

Davenport-Perth Neighbourhood Centre and Community Health Centre (DPNCHC) is a multi-service organization that provides various health and community programs and services for a diverse community. We are seeking a physiotherapist to provide a comprehensive range of services across the continuum of care, including assessment, treatment, health promotion, supportive care and referrals. The successful candidate will possess a range of clinical experience (including chronic disease management) with different populations using a variety of modalities.

We are expanding our services to deepen our work with the Mid-West Toronto Ontario Health Team (MWT-OHT) and the Primary Care Network. This staff will be assigned to the MWT-OH Interprofessional Primary Care Team (IPCT) and will practice at a community location (340 College St., Toronto, ON). This role will accept referrals from community-based physicians in the Family Practice Network (MWT-FPN). The successful candidate will be expected to work one evening a week, some weekends, and work independently and as part of a team.

Abbreviated Responsibilities:

Develops care plans by assessing, developing, implementing and evaluating treatments for clients, primarily with acute and chronic musculoskeletal dysfunction;
Ensures continuum of care is maintained by educating clients and family members, if appropriate, about the management of health care problems and re-evaluating physiotherapy programs considering the context of the client’s home environment (home visits not required);
Provides consultation and capacity building to other centre staff and community agencies working with clients who may need physiotherapy care;
Develops, implements, monitors and evaluates education and health promotion programs for individuals, families and the community to foster self-help, mutual aid, and capacity building in the area of prevention or management of neurological and musculoskeletal problems according to community needs;
Develops and maintains formal and informal networks within the community to optimize health care service provision and address the needs of the community;
Maintains professional competence through appropriate continuing education methods;
Maintains physiotherapy equipment, supplies and educational material for professional and client use;
Maintains complete and accurate client records, assessment notes and required correspondence;
Participates in chart reviews and case conferences and makes appropriate internal and external referrals.

Qualifications:

Master of Science in Physiotherapy from a recognized university or an equivalent combination of relevant education and experience;
Registered under the College of Physiotherapists of Ontario;
Eligibility for membership with the Canadian Physiotherapy Association;
Thorough knowledge of and proficiency in current physiotherapy techniques;
Possesses strong exercise prescription skills;
Experience in a variety of health care setting with experience in the community an asset;
Broad range of clinical experience (including chronic disease management) with different populations using a variety of modalities;
Demonstrated ability to work effectively in an inter-professional team environment;
Demonstrated ability, commitment to and knowledge of community health care;
Demonstrated commitment to continuing education;
Experience in program development, implementation, monitoring and evaluation;
Excellent interpersonal skills with clients and adept at working with a marginalized population.
Excellent communication, decision-making, problem-solving, conflict management and time management skills;
Training in manual therapy, acupuncture and/or dry needling considered an asset;
Proficiency in the use of computers and various software applications;
ADP authorization is considered an asset;
A second language relevant to catchment area populations, such as Spanish or Portuguese, is an asset.

Salary Range: $71,656-$83,550 per annum, plus extended health benefits and Healthcare of Ontario Pension Plan (HOOPP). This position is part of the bargaining unit.
